Track & Field State Meet Scratch and Name Change Forms

For changes to be reflected in the State Meet Program, changes MUST BE submitted no later than 4:00PM on Friday, May 3rd. All scratch/relay forms must be submitted by 12:00 PM on May 9th. Any relay changes needing to be made after the 9th must fill out a Relay Scratch Form at the State Meet.
