Medical History
The medical history form and pre-participation physical are important tools for student-athlete safety. It is critical that parents and students answer questions on the medical history form carefully, and accurately. The questions are designed to determine if the student has developed any condition which would make it hazardous to participate in an athletic event.
The Medical History Form is required annually, and a pre-participation physical evaluation is required every other year for all student-athletes. Any concerns raised by the medical history form must be further evaluated by a physician before an athlete can participate.
The Medical History form required each year includes the 12-question heart screening recommended by the American Heart Association. If a student or parent answers "yes" to any of these questions, additional testing must be completed by a doctor before a student can participate.
